Diagnostic Value of Serum Alkaline Phosphatase ¥á_1-Isoenzyme ¥ã-Glutamyltransferase ¥á-Isoenzyme in Primary and Metastatic Liver Cancer

Abstract
The electrophoretic analysis of serum alkaline phosphatase and ¥ã-glutamyltransferase using cellulose acetate membrane was performed on 223 patients for the assessment of the diagnostic value of the electrophoretic results and othere tests in detection of primary and metastatic liver cancer. There were 59 patients with primary liver cancer, 38 with metastatic liver cancer, 82 with other cancer and 44 with benign liver disease. The results obtained were as follows: 1. Alkaline phosphtase ¥á_1-isoenzyme appeared to have the greatest diagnositc value in primary and metastatic liver cancer with 85.2% agreement, 22.7% false-negative, 8.7% false-positive, and 87.2% predictive value of a positive test. 2. Determination of ¥ã-glutamyltransferase ¥á-isoenzyme was also a valuable test, though less sensitive tan alkaline phosphatase ¥á_1-isoenzyme in primary and metastatic liver cancer with 79.0% agreement, 30.4% false-negative, 7.4% false-positive, and 93.2% predictive value of a positive test. 3. Among other tests, ¥ã-glutamyltransferase only had a diagnostic value with 78.5% agreement, 14.4% false-negative, 27.0% false-positive, and 70.9% predictive value of a positive test. 4. In this study, the positive incidence of alkaline phosphatase ¥á_1-isoenzyme and ¥ã-glutamyltransferase ¥á_1-isoenzyme were very highly correlated with an agreement of 94.3%(p¡Ö0). 5. On the basis of the relative diagnostic value of the tests by discriminant analysis, determination of alkaline phosphatase ¥á_1-isoenzyme was the most valuable test with the highest discriminant coefficient, 0.78025. 6. In primary liver cancer, there were no statistical difference between serum ¥á-fetoprotein level and the occurrence of alkaline phosphatase ¥á_1-isoenzyme and -glutamyltransferase ¥á_1-isoenzyme, respectively (p=0.94, 0.97).
